Completing this form is the first step to acquire an incentive payment when you complete your project. You must submit a Pre-Application prior to submitting your project. You can complete an online form, or you can use a printable PDF form (PDF, 64KB). Click for details on getting started.

Remember, you must document the equipment installed and the costs associated with each measure you install. To receive payment under the Easy Upgrades incentive program, you must also file your Final Documentation (PDF, 65KB) with the appropriate worksheet(s) and any other required materials within 90 days of your Pre-App acceptance.
